When you have downloaded the form, you can complete it in Word, or you can print the form and complete it by hand.

or email the Word document as an attachment to: manager@lilliputlodge.co.uk
Notes on how to complete the form using Word
- Open the form in Word. You will see pale-gray shaded boxes in each blank section, and the first box (in the Name section) will be highlighted.
- Just start typing. The shaded box will expand to contain your type. The shading does not print, although it shows up in Print Preview.
- To move from box to box, press the Tab key.
- To skip boxes, use the mouse and click in the box you want.
- The Signed box has been left blank, so that you can print the completed form and then sign it yourself.
- You can use the Enter key to start new lines, in the Address section for example. You can delete and re-type what you have entered. As you can only type where the shading is, you can’t delete or alter the actual wording of the form.
- When you have completed the form, print it, sign it, and post it to Caroline Neale at the above address,
- or email the Word document as an attachment to manager@lilliputlodge.co.uk
